The Department of Children and Families announces the release of the

Nomination Forms for the 2005 Governors Peace At Home Awards: Stopping

Domestic Violence. The Peace At Home Awards recognizes the work that is

being done in the prevention of domestic violence throughout the state.

 
As awareness of domestic violence has grown over the years, so has the

number of remarkable individuals and organizations who are making a

difference in our state. Each year, Governor Jeb Bush honors individuals and

organizations in Florida for their outstanding accomplishments and

contributions in the areas of Children’s Services, Survivor Support,

Justice System, Health Care, Public Awareness and Education, and

Research.

Please join us in identifying candidates for the 2005 Peace at Home Awards.

For your convenience the nomination packet has been attached to this

message. The deadline for submitting nominations is February 4, 2005.

GOVERNOR’S PEACE AT HOME AWARDS:
Stopping Domestic Violence

As awareness of domestic violence has grown over the years, so
has the number of remarkable individuals and organizations that
make a difference in our state. Domestic violence is a crime that
seriously impacts our communities, causing traumatic
consequences for the victims and their families. Through vision,
action and leadership, numerous people and organizations have
improved the lives of domestic violence victims and their families.

Each year, through the Governor’s Peace at Home Awards,
Governor Jeb Bush honors exceptional citizens and organizations
for their outstanding accomplishments and contributions in the
areas of services to children, survivor support, justice system
programs, health care, research, public education and awareness,
and overall.
The Domestic Violence Program Office within the Florida
Department of Children and Families coordinates the submission
of nominations for selection by the Governor.

On April 11, 2005 at 10:00 a.m. on the 22nd Floor of the
Capital, Tallahassee, the Governor will host an awards ceremony
to honor the award-winning nominees.
Please join us in identifying candidates for the 2005 Governor’s
Peace at Home Awards.

Award Categories

Awards will be presented in the following categories to persons,
organizations or programs that have:

Children’s Services

Developed prevention programs or enhanced support for
children who have suffered from the trauma of domestic
violence.

Survivor Support

Promoted greater self-determination and empowerment of
survivors through exemplary programs or service.

Justice System

Developed programs or services to prevent domestic violence,
protect the public, or stop the perpetrators of domestic violence.

Health Care

Provided health or dental care through volunteer service to
domestic violence victims and their children.

Public Awareness & Education

Promoted education and awareness of domestic violence through
communications, media, curriculum, or other activities that have
advanced a greater understanding of the problem.

Research

Conducted research and evaluation on domestic violence to
expand the field of knowledge and encourage new solutions.

An Overall award will be presented to an individual or
organization whose efforts towards ending violence in the family
far exceeds the requirements of their employment, organizational
mission, or program activities. Such an individual, organization or
program is generally known throughout the community for their
“beyond the call of duty” activities in assisting domestic violence
survivors. The winner will be selected from the nominees in all
categories.

Dear Friends:
In your community there are those who play an
important role every day in an effort to end
domestic violence. They may be a volunteer at a
local domestic violence center, a doctor or dentist
who provides free care to victims, a law
enforcement officer, judge, or state attorney who
has protected the rights of victims and brought
abusers to justice; or, a local service organization
that has promoted domestic violence education
and awareness. This is your opportunity to help me
recognize the individual or organization for the exemplary
service provided to domestic violence victims.

I invite you to submit a nomination for the Governor’s Peace
at Home Awards: Stopping Domestic Violence in one or
more of the six categories—Children’s Services, Survivor
Support, Justice System, Health Care, Public Awareness and
Education, and Research. Recipients will be honored as part
of the state commemoration of Victims’ Rights Week in
Tallahassee on April 11, 2005.

Nomination Submission
Guidelines

1. Each nomination form must include the nominee’s full
name and all requested contact information. If an
organization is the nominee, provide the name of the
individual who would accept the award. Also, it must
include all of the nominator’s contact information.
2. A nomination letter must accompany each nomination
form. If you, the nominator, are affiliated with an
organization your nomination letter should be submitted on
official stationary and contain your signature, as well as
your position/title. Anonymous nominations will not be
accepted.
3. The nomination letter should not exceed two singlespaced,
single-sided, letter-sized (8 ˝” x 11") pages.
Provide as much as you know about your nominee’s
accomplishments and services in support of domestic
violence victims. The letter should address each of the
following questions:

· What activities has the nominee accomplished in the
area nominated on behalf of domestic violence victims?
· How long has the individual or organization been
providing services?
· How does the nominee’s contributions qualify as
“beyond the call of duty”?
· How have the actions of the nominee influenced
attitudes, policies, or practices in domestic violence
areas?
· How has the nominee generated support and
interest of the local community for domestic
violence victims?

4. Three (3) letters of reference from professionals who
have worked with the person or organization are
necessary. Be sure that references include all pertinent
contact information. Letters must be submitted with
the nomination package.
5. The resume of any person nominated is required.
6. Supporting documentation such as program materials,
articles, etc. may be attached to this form, but please be
concise and limit your materials to key items only.
Supporting material must not exceed standard 8˝ x 11
inches. Videos, bound publications, and notebooks should
not be submitted. Any material supplied to support the
entry will not be returned. If you are faxing your
nomination package, you may mail the supporting
materials separately, but please note on the nomination
form that “supporting documents are to follow by mail.”
Seven (7) copies of the supporting documentation must be
provided.

Eligibility Guidelines

1. You may nominate one or more worthy candidates,
including:

· Individuals
· Organizations
· Network or coalition of individuals or programs.

2. Candidates nominated, but not selected, in previous years
are also eligible for consideration.
3. You may nominate yourself, your program, or organization.
4. Nominees are not notified directly unless chosen as a finalist.

Deadline for Submitting

Nominations

If you are mailing the nomination form and supporting
materials, it must be postmarked no later than February 4,
2005 and mailed to the address below. If faxing, it must be
received no later than February 4, 2005 and faxed to the
number below.
